Join us for our Social Determinants of Health & PA Navigate Cohort Call—a collaborative and interactive space for staff working to address the social factors that impact health and well-being.

This first call of 2026 will serve as an informal kickoff to the year. Rather than focusing on a set presentation, we’ll engage in open dialogue to better understand the topics most important to you, the key challenges you’re navigating, and the types of expert speakers or resources that would be most valuable in future sessions. Your input will directly shape our rotating topics for the year ahead—ranging from community partnerships and program implementation to strategies that improve outcomes.

Bring your questions, share your experiences, and exchange ideas with peers who understand the unique challenges and opportunities in this work. Whether you’re seeking guidance, offering insights, or simply listening in, this is a space to connect, learn, and help co-create a cohort experience that strengthens your impact throughout 2026.
