Join the Community Action Association of Pennsylvania (CAAP), in partnership with Central PA Connect Health Information Exchange (HIE), for an interactive information session on PA Navigate—the statewide platform designed to strengthen connections between healthcare providers and community-based organizations. This session will provide an overview of PA Navigate, the stakeholders involved, and how the platform supports coordinated referrals and resource connections to address social determinants of health. Participants will learn how PA Navigate can be used to identify community resources, streamline referrals, and improve collaboration across sectors. The session will also highlight how PA Navigate can be leveraged to promote volunteer opportunities that help individuals meet work or community engagement requirements and maintain eligibility for SNAP and other public benefits.
