August 2026 Health/PA Navigate Cohort Call

We're excited to welcome Maria Welch, Director of Community Health and Social Needs at Geisinger, and Nicole Matrey, Senior Health Services Specialist at Geisinger, for our upcoming PA Navigate/SDOH cohort call.

This session will dig into building shared language between CBOs and healthcare organizations — and what it really takes to create meaningful, lasting partnerships. Maria and Nicole will walk through Geisinger's own journey: how it started vs. how it's going, sharing honest lessons learned along the way. We'll also hear firsthand insight from CBOs actively involved in this work, offering a grounded, real-world perspective on what these partnerships look like in practice.

Whether you're just beginning to bridge the gap between healthcare and community-based work or looking to deepen an existing partnership, this conversation will offer practical takeaways for strengthening collaboration on both sides.
